SECTION 5: HEALTH & SAFETY

The health and safety of all participants is our top priority.

If a participant should have a low-grade fever or exhibits any symptoms of illness, please stay home. Participants exhibiting any symptoms of illness will be sent home.

For the safety of all participants, children will remain inside Lake Cook Music Studio until they are picked up by a parent or legal guardian. If a child needs to use the restroom, which is located down the hall from the studio, they will be instructed to notify a staff member before leaving the room.

Please Note: LCMS staff will only release a child to the parent(s) or guardian(s) listed on this form unless prior written authorization is provided by the child’s legal parent or guardian. (See below.) For security purposes, LCMS may request to see a valid photo ID at pickup to verify the identity of the individual picking up the child.

Lake Cook Music Studio is located in the Spoerlein Commons Mall on the far east end of the 2nd floor, just above Rosati's Pizza.

If you have any questions or need assistance finding the studio, please call or text the LCMS main phone number at (224) 633-6611.

The workshop will begin promptly at 3:00 PM, with doors opening for participants at 2:45 PM.

The session will conclude at 5:00 PM, and parents or guardians are expected to pick up their child promptly at that time. If you anticipate being late, please contact LCMS as soon as possible to inform our staff.
